Make a request to be considered to move hospital

Before you start

Please be aware that by making a request to move your care to another hospital, your entire pathway of care will be moved. This includes all appointments, any required treatment / surgery and any follow up care.

Every effort will be made to identify an alternative hospital, however there may be circumstances in which it is not clinically appropriate to move to a different hospital or alternative capacity is not available. In these circumstances you will remain on the waiting list with your existing hospital and will be contacted as soon as an appointment becomes available.

Please only submit a request if you are willing to transfer to a different hospital for your entire care pathway.

How to make a request

The easiest way to make a request is to use this online system.

By clicking 'Continue' below, we will ask you to log in using your NHS login account to verify your details. If you do not already have an NHS login account you can create one by following this link and selecting 'Create Account' - About NHS login

If you are unable to use the online system, please contact the NHS Patient Choice Helpline on 0345 450 6166 who will be able to submit a request for you.

Helpline opening hours are weekdays 9am to 6pm; closed on bank holidays.

Before you continue

To make a request you will need the following information:

  • The name of the hospital you have been referred to or are currently under the care of
  • The specialty you have been referred to or are under the care of

Please note - this information may be found on the text/letter/email containing the information for this site, your referral letter, or other letters from the hospital.

In addition, we will also need to know:

  • The distance you are willing to travel to attend a different hospital.

If you have received an appointment for your current hospital within the next 8 weeks, we would advise you to remain with your current hospital for your care.
