Heeley Plus Primary Care Network are a group of 7 practices in Sheffield, who serve a population of around 40,000 patients. We are an active network who is currently engaged in work with our local health and social care organisations to support and develop care models for our patients and community.

As a primary care network, we work closely with Heeley Trust, a local community organisation.

Our Vision

Heeley Plus aims to unify Health and Social care for all involved (staff, patients, NHS, residents…). We believe outcomes can be improved by working together in holistic manner to improve care for all.
