NHS Type 2 Diabetes Path to Remission Programme

This NHS programme provides a low-calorie diet treatment for people who are overweight and living with type 2 diabetes.The programme is based on the DiRECT study which showed that this approach could help people lose weight, improve their diabetes control, reduce diabetes-related medication and even achieve remission (whereby blood glucose levels return to normal without the need for medication).

From the 1st February 2024, the T2DR programme is changing to a 1:1 in person (face to face clinics) or digital delivery model. This means the programme will no longer be run as group sessions (except those who have already commenced a group programme).

Please note although this search is titled the Low Calorie Diet Search, this is still the recommended search method to identify people who may be eligible for the T2DR programme.

Referrals to the NHS T2DM path to remission programme are open to all areas in Somerset. 

Participants are now offered a choice of either digital support via an app and virtual health coach or the option of in-person 1:1 health coach delivery in a nearby location. 

From receipt of referral, participants should be able to start the programme within a month.

Important note: The medication adjustment form must be completed, even if they are not on any medications or no adjustments are needed.

Drop in sessions

Book a drop in session with Debbie Fry, regional representative for the T2DR provider Momenta.  

30 minute sessions can be booked with a choice of days and times. 

These sessions are useful to have any questions you have answered or if you wish to simply learn more about the programme..

PCN T2DR events for patients

We are offering large practices and/or clusters of smaller practices and PCNs the opportunity to offer their potentially eligible patients to a patient information event which could be run either at the practice or in a community venue. 

 

At these events the project lead officer (also a dietitian) and a health coach from the T2DR programme would provide an overview of the programme, including:

  • Background
  • Benefits
  • Eligibility - who is suitable
  • Information about the programme format, delivery and support
  • Soups and shakes to sample
  • Opportunity to ask questions
  • Signposting of what to do next if they would like a referral

The benefits of these events include:

  • Patients are fully informed about the programme and the commitment required
  • They are aware of the potential benefits
  • They have an opportunity to trial the soups and shakes
  • They can check their eligibility
  • They have their questions answered and assess whether the programme is right for them or not
  • This event supports primary care by ensuring minimising those initial queries from patients and ensuring those who seek a referral are fully informed about the programme

What do we need to do to take part?

  1. Run the search (either in your practice or at PCN level) to gauge numbers of potentially eligible patients (bear in mind approx 20-25% of those may be interested)
  2. If your practice/PCN has at least 100 potentially eligible people (e.g. this would mean 20 who may be interested in attending an event), contact Lesley.Harper1@nhs.net - a venue can then be booked and tailored invitations sent to potentially eligible patients

“We have had some wonderful feedback for our patients currently on the LCD programme with some excellent weight loss results. I have seen 3 people very recently (just after the 12 wk TDR phase has been completed), 2 of which now have an HbA1c <48 and the other one is much improved at 49.  They have all told me how well run the programme is and are grateful for the opportunity to turn things around”
