We have a dedicated, experienced Referral Generation and Partnership Officer (RGO) who offers support and data to help you provide diabetes prevention opportunities to your patients. We can arrange for your local Referral Generation Officer (RGO) to visit you and your team to discuss how we can help in more detail, and how we can work collaboratively. We’re also available to represent Healthier You at your outreach activities or events.
After more detailed information about your referrals, or an overview of our services? We’ll share personalised information regarding referral routes for your patients, as well as referral data and KPI outcome data. Clinical meetings work particularly well to encourage referrals from complex patients who may require your support to make a referral.
We offer a range of leaflets and posters to help promote Healthier You to your patients.

Detailed steps to help you achieve QOF points through your referrals, and information on how many of your patients are eligible, and how we can help you to reach those patients not yet in service.
