HEE Midlands Advancing Practice Trainee Data Collection Form

Dear Advancing Practice Trainee,

The Midlands Faculty of Advancing Practice is required to record current NHS England (formerly HEE) supported Advancing Practice trainees in order to better support trainees by keeping track of who is currently within the programme and who requires funding. The Midlands Faculty of Advancing Practice  will record personal and sensitive information as well as training programme information. 

We therefore require you to complete an up to date record of your training information via this form. Please note, should you not complete the form, the funding for subsequent years of training may be impacted.

The details provided via this form will be used by NHS England staff only. 

• Your personal data will be securely stored by NHS England as the data controller, and by Jisc Online Surveys as the data processor licensed by NHS England.

• Personal data that we gather as part of this process will only be accessed, seen and used by NHS England staff and Primary Care Training Hubs who need access to the personal data to fulfil their role.

• We will not share your personal data with any third-party organisations that are not involved in your training.

• Where the data is used for analysis and publication, it will be anonymised to prevent you from being identified.
