The NHS Diabetes Prevention Programme (NDPP) is a face-to-face and tailored behavioural change programme available to people who are found to be at risk of Type 2 diabetes on their blood tests (HbA1c 42-47mmol/mol) or patients with a past diagnosis of gestational diabetes mellitus (GDM). It has been provided in NCL by Living Well Taking Control (LWTC) since August 2022.

The nine-month programme offers patients advice on diet, exercise and healthy lifestyle through a range of support, including sessions, motivational interviewing, and digital elements, including an app with videos. It is delivered face-to-face (group sessions), or digitally (one-to-one app-based coaching) by LWTC's digital partner Liva Healthcare. More information can be found on LWTC's website in Resources.

Many people who are eligible for the programme do not speak English as a first language and, as such, should not be referred through mail-shots.

IT Tools are available to support the referral of patients to the programme. NDPP eligible patient searches are deployed via NHS NCL ICB enterprise search and reports.

NHS Diabetes Prevention Programme: Gestational Diabetes Mellitus (GDM) Pathway

Living Well Taking Control (LWTC) would like to promote it's new NHS Diabetes Prevention Programme (NDPP) self-register pathway for GDM patients.

Historic GDM patients can now self-register on the NDPP with LWTC via the online self-registration form.

The key pieces of information required for a patient to fill in on the GDM form is their personal details, NHS number and GP surgery info. More information and the NDPP GDM pathway can be seen in the GDM specific leaflet and on the Gestational Diabetes website.

HealtheIntent dashboard

A HealtheIntent diabetes prevention dashboard is available for primary care use. The diabetes dashboard can be found on the HealtheIntent website, under the Reports tab, entitled NDPP – National Diabetes Prevention Programme Dashboard.

The dashboard can be used to case-find eligible patients to be referred to the NDPP. The dashboard can also be used to track eligible patient progress, including invitations, referrals and programme completions.

CPD-accredited training

The Royal College of GPs (RCGP) has developed an e-learning module on the NDPP for GPs, practice nurses and other practice staff. 

The free 30-minute module is CPD-accredited and covers: 

• what non-diabetic hyperglycaemia is
• what the NDPP is
• how it works
• who is eligible.

It can be accessed by creating a free account on the RCGP learning site. 

Centralised referrals project

The NDPP has recently launched a health inequalities project targeting patients at risk of developing diabetes who are living in areas of high deprivation in NCL.

Through this centralised referrals project, patients eligible for the NDPP who are living in the most deprived areas in NCL will be contacted by iPlato and invited to join the programme. iPlato will be contacting eligible patients via text message and letter to provide patients with their eligible HbA1c reading and instructions on how to register for the NDPP.

This project was signed off by the NCL Information Governance board in December 2022, and practices were contacted by Steve Durbin.

Eligibility Criteria

Inclusions

• aged 18+
• women with a past diagnosis of gestational diabetes mellitus (GDM) and a normoglycaemic blood reading within the last 12 months (see Downloads for more information on GDM inclusion from NHS England)
• registered with an NCL STP GP practice
• not pregnant
• able to take part in light/moderate physical activity
• HbA1c between 42-47 mmol/mol (6.0-6.4%) or Fasting Plasma Glucose between 5.5-6.9 mmols/l within the last 12 months.
